InterpretationProphylactic dose: Trough Levels should be > 0.7mg/L. Treatment dose: Trough Levels of <1.25mg/L are subtherapeutic. If levels are below this - ensure drug is taken with fatty food, consider increasing dose (max 800mg/d) or examining concomitant medications. As yet, the level at which posaconazole becomes toxic has not been defined.
