DescriptionBNP is a natriuretic peptide hormone secreted by the left ventricle of the heart. The N-terminal Pro-B-type natriuretic peptide (NTproBNP) is an assay used as an aid in the diagnosis of individuals suspected of having congestive heart failure. The test is further indicated for the risk stratification of patients with acute coronary syndrome and congestive heart failure. The test may also be used as an aid in the assessment of increased risk of cardiovascular events and mortality in patients at risk for heart failure who have stable coronary artery disease. NT-proBNP is used at LTH because it is more stable in vitro than BNP itself.
IndicationIn a breathless patient with or without peripheral oedema, a normal BNP test can be used to rule out heart failure as the underlying cause. A positive BNP test does not confirm heart failure and echocardiography is indicated.

InterpretationAll ages: <400 ng/L = low risk (NT-proBNP indicates low probability of Cardiac Failure. Consider alternative diagnosis); 401-1999 ng/L = intermediate risk (NT-proBNP is associated with heart failure in fewer than 25% of patients. Please review the patient and if heart failure is still suspected consider a referral for NT-proBNP-guided echocardiography); >2000 ng/L = high risk (NT-proBNP is associated with a 50% chance of heart failure, and patients with this level should be referred to the heart failure clinic.).
