DescriptionPEMPHIGUS : Autoantibodies directed against desmogleins on the cell surface of epidermal keratinocytes. BULLOUS PEMPHIGOID : Autoantibodies directed against hemidesmosomes in the basement membrane. Both are always tested.
IndicationThese are rare, but serious autoimmune skin diseases. Bullous Pemphigoid - tense blisters, mucosa rarely involved. Pemphigus Vulgaris - flaccid blisters, mucosa often involved.
Additional InfoImmunofluorescence of a skin biopsy should also be performed (by Histopathology) as both assays can give false negative results.

Interpretation70-90% of patients with Pemphigus Vulgaris/ Bullous Pemphigoid have antibodies. Antibody levels may correlate with disease activity for Pemphigus only.
