Autoantibody Tests
TEST SAMPLE
(clotted blood)
Turnaround TIME
( working days)
COMMENTS
Acetylcholine receptor
5-10 ml
28
Referral to outside lab
Adrenal autoantibodies
5-10 ml
7
Immunofluorescence
Autoantibody Screen
5-10 ml
7
Using ‘Autoantibody screen’ as a request can be very misleading. It is recommended that relevant antibodies be requested. Testing for ANA (Hep-2 cells) is the only investigation undertaken unless relevant clinical information or request is received.
Cardiac
5-10 ml
7
Immunofluorescence
Cardiolipin 5-10 ml
7
IgG only
Centromere
5-10 ml
7
Detected on Hep-2 cells as part of ANA testing
Cyclic citrullinated peptide CCP 5-10 ml 7 NICE CG79. Where these is a strong suspicoon of RA, but RF is negative, consider testing anti-CCP Ab.
Tissue Transglutaminase (IgA) for diagnosis of coeliac disease
5-10 ml
7
All positive samples will also be tested for IgA anti-endomysial antibodies
DNA (double stranded)
5-10 ml
7
Measured on the Bioplex 2200 as part of the ANA screen.
Epidermal: Pemphigus or Pemphigoid
5-10 ml
7
Reported as positive or negative then titred
Extractable Nuclear Antibodies 5-10 ml 7 Measured on the Bioplex 2200 as part of the ANA screen. Ab to Ro60, R52, La, Sm, SmRNP, RNP68, Centromere, Jo-1, Scl-70, chromatin and Ribosomal P antigens can be detected.

Ganglioside
5-10 ml
28
Referred to outside lab
Gastric Parietal Cell (GPC)
5-10 ml
7
Done as part of multi-tissue block OR together with AMA, SMA, LKM
Glomerular Basement Membrane* (GBM)
5-10 ml
7
URGENT RENAL by prior arrangement
Glutamic Acid Decarboxylase (GAD)
5-10 ml
14
ELISA
Only undertaken when ANA positive
Insulin
5-10 ml
28
Referred to outside lab
Jo-1
5-10 ml
7
See ANA/ENA entries
Liver Kidney Microsomal (LKM)
5-10 ml
7
See comments on GPC
Mitochondrial (AMA)
5-10 ml
7
See comments on GPC
Neutrophil Cytoplasmic (ANCA)*
5-10 ml
7
URGENT RENAL CASES by prior arrangement. Requests for ANCA are carried out by indirect immunofluorescence according to international guidelines when positive, quantitative testing for MPO and PR3 are done automatically.
Nuclear (ANA)
5-10 ml
7
ANA screen in now routinely done on the Bioplex 2200 multiplex analyer. The laboratory retain the ability to measure ANA by Immunofluorescence using Hep-2 cells.  Pattern reported but not titre.
Ovarian
5-10 ml
7
Immunofluorescence

Pancreatic Islet Cell (PICA)

5-10 ml
7
Immunofluorescence
Pituitary Antibody 5-10 ml 28 Referral to outside laboratory
Purkinje Cell
5-10 ml
7
Requests for Hu, Yo, Ri antibodies will have initial tests done for anti-Purkinje cell (PC) antibodies. Negative PC antibodies precludes positivity of the above antibodies.
Rheumatoid Factor
5-10 ml
7
 By Nephelometry
Ribosomal
5-10 ml
7
 See ENA/ANA entries
Scl-70
5-10 ml
7
See ENA/ANA entries
Smooth Muscle (SMA)
5-10 ml
7
See comments on GPC
Striated Muscle
5-10 ml
7
Immunofluorescence
Voltage-gated Calcium or potassium channel
5-10 ml
28
Referred to outside lab

* Turnaround time for ENA screen and DNA includes time for initial ANA assay. Time for specific ENA includes ANA and ENA screen. Turnaround time for individual tests is 5-7 days.
